Number Porting Authorization
This form authorizes BaronTEL to transfer your existing phone numbers from your current provider to our service.
- Required for all number transfers
- Must match your current provider's records

911 Service Acknowledgement
This form confirms your understanding of how VoIP 911 services work and the important differences from traditional landline 911 services.
- Required for all new services
- Confirms your service location

Form Submission Instructions
Follow these steps to properly complete and submit your forms
- Download Both Forms - Click the download buttons above to get the PDF forms.
- Complete All Fields - Fill out all required information on your computer. Make sure your information exactly matches your current provider's records.
- Print and Sign - After filling out the forms, print them and sign where indicated with a physical signature (digital signatures are not accepted).
